A Health Home isn’t a place, it’s a service that provides free comprehensive care management services to Medicaid recipients to make certain their medical, mental health, and substance use disorder needs are being met.
A Health Home is a Care Management model that provides individuals with access to a wide network of physical and behavioral health providers and community services. All of an individual’s service providers communicate to monitor and provide linkages with one another, to address an individual’s unique social, physical, and behavioral health needs in a comprehensive way. This is done primarily through a “care manager”. Health Homes are known to improve the health outcomes of individuals with chronic conditions (including mental health and substance use) and reduce inappropriate use of inpatient and emergency room care. Health Home services are provided through a network of organizations – providers, community-based organizations and Care Management agencies. When all the services are considered collectively they become a virtual “Health Home.” This network of organizations provides individuals and families the opportunity to select a New York State authorized Care Management agency within their region.
Health Homes of Upstate New York (HHUNY) is a management services organization (MSO) formed in 2013 that provides administrative support and governance to four affiliated Health Homes and over 80 Care Management agencies in New York State. BestSelf Behavioral Health is one of the four affiliates of HHUNY and also one of the 80 Care Management agencies in New York State. As a Health Home, BestSelf has 17 community partners, known as Care Management agencies, in order to provide adequate access to individuals and families across Erie, Niagara and Wyoming Counties.
