Health Home – Adult

The BestSelf Health Home provides free, comprehensive care management services to qualified Medicaid recipients to ensure their medical, mental health and substance abuse needs are being met. Our Health Home can assist members in a variety of areas, including: developing a personalized Care Plan; activities to improve and maintain health; resources for finding affordable housing, legal assistance, food and other essential needs; successfully transitioning from one type of care to another; and more.

BestSelf Health Home Services is a partner with Health Homes of Upstate New York (HHUNY).

Qualifications

Two or more chronic health conditions, such as asthma, diabetes, heart disease, mental health condition or substance use disorder

Significant Mental Illness

Living with HIV/AIDs

Medicaid recipients in Erie County

Individuals who have both Medicaid and Medicare are also eligible

Individuals who are members of a Managed Care Organization are also eligible

Our Highly Qualified Team have received intensive training in PCIT & TCIT
