What is the SCN Program?
The Social Care Navigation (SCN) Program helps connect individuals and families to essential resources that support overall health and well-being, including assistance with housing, food access, transportation, and utilities.
Through this program, BestSelf is expanding access to equitable, community-based care for underserved populations across Western New York. By addressing social factors that can impact health, such as housing instability, poverty, and barriers to care, the program aims to reduce health disparities and improve quality of life for Medicaid members facing behavioral health and social challenges.
In partnership with WNYICC and other community organizations, BestSelf continues to strengthen its role as a regional leader in integrated health and social care, helping individuals access the support they need to thrive.
How the SCN Program plays a critical role:
- Brings together clinical teams, community partners, and care coordination staff to reduce barriers to care and improve overall health and wellness outcomes.
- Directly supports BestSelf’s mission to deliver accessible, equitable, behavioral health and community support.
- Strengthens BestSelf’s role as a regional leader in integrated health and social care, collaborating with WNY Integrated Care Collaborative (WNYICC).
Who is eligible for the SCN Program?
- Members with substance use disorder and/or serious mental illness
- Members with intellectual and developmental disabilities
- People recently released from incarceration who are managing chronic health conditions
- Children under 18 with qualifying chronic health conditions
- People who are pregnant or have recently given birth
- People who frequently use emergency room or hospital services
- Members enrolled in a Health Home
